What You'll Be Doing

Main Function :

• The Counselor is responsible for assessing, counseling, intervening, implementing, and educating the patient and family / significant others to facilitate movement throughout the continuum of care.
• Makes psychosocial assessments appropriate to the patient's age and diagnoses.
• Appropriately prioritizes cases according to assessed needs.
• Written assessments completed within 24 hours of assessment.
• Documented assessment demonstrates understanding of patient's functional status and psychosocial situation.
• Assessments are completed based on information obtained from patients, families / significant others, staff, and interdisciplinary rounds as needed.
• Assessment prepares for the possibility of changes in the patient's condition / status.
• Provides supportive counseling to patients and families.
• Coordinates discharge planning services.
