• Assess / monitor physical, emotional, and psychological needs of patients\n
• Create hospice care plans that align with the patients wishes and goals\n
• Direct nursing care : administering medications, treatments, and interventions\n
• Provide pain and symptom management\n
• Educate and support the patient's family and caregivers\n
• Collaborate with interdisciplinary team\n
• Maintain accurate and timely documentation\n

• Graduate of an accredited nursing school with current licensure to practice in state of operation\n
• One year nursing experience required, 3+ years preferred\n
• Experience in hospice or a similar setting preferred\n
• Knowledge of the hospice philosophy of care\n
• Commitment to clinical and documentation excellence\n
