Nurse Practitioner

Pride Health is hiring a Nurse Practitioner in Rochester, NY! (Contract)

Work between 2-3 Long Term Care settings in Rochester, NY

Schedule : Monday-Friday (8am-5pm)

Contract : 6 month contract to hire

Pay Rate : $66-70 / hour

Specific Duties :

Provide specialized care to high-risk health plan members

Work as part of an interdisciplinary team

Conduct comprehensive assessments

Manage acute and chronic medical and behavioral conditions

Ensure accurate documentation, including ICD-10 coding

Communicate and collaborate with the care team

Facilitate advanced illness and care planning discussions

Offer counseling and education to patients and caregivers

Must have qualifications / experience :

Nurse Practitioner or Physician Assistant

NP Certified in one of the following (FNP, ANP, GNP)

Access to transportation

1+ year of experience (LTC, SNF, Geriatric preferred)
