Join a team that values compassion, quality, and the difference you make every single day.\n\nYour Impactful Role as an LPN : \n\nAs a Licensed Practical Nurse with Loyal Home Care Services, you'll be at the center of personalized care plans - delivering top-tier support and forming trusted relationships with clients and families.\n\nResponsibilities : \n

• Deliver skilled nursing care based on the Plan of Care and within scope of MA Nurse Practice Act\n
• Support and carry out care plans initiated by the Registered Nurse\n
• Provide education and guidance to patients and families on condition management and at-home care\n
• Monitor health status, document accurately, and report updates to the clinical team\n
• Collaborate with RNs and coordinate additional services (PT, OT, HHA, MSW, etc.)\n
• Use medical equipment safely, maintaining infection control standards\n
• Stay current on best practices through ongoing clinical development\n
• Be an advocate and source of encouragement for your patients\n\nWhat You'll Need to Join Us : \n
• Active LPN license in Massachusetts\n
• Graduate of an accredited LPN or vocational nursing program\n
• Minimum of 3 years nursing experience (home health or med / surg preferred)\n
• Current CPR / BLS certification\n
• Reliable transportation and valid driver's license\n
• Up-to-date health certificate & TB test\n
• Fluent in English (spoken and written)\n
• Demonstrated knowledge of HIPAA & patient confidentiality\n
• Must be 18 years or older and authorized to work in the U.S.\n\nPerks & Benefits\n
