Compact State Nurse Case Manager

A company is looking for a Case Manager Triage Nurse.

Key Responsibilities

Conduct comprehensive health assessments and gather relevant information to develop a holistic understanding of members' health needs

Provide telephonic and digital education, nursing interventions, and coaching on health topics, while empowering members to manage their health

Coordinate with healthcare providers and community resources to facilitate access to necessary services and monitor individuals' health status

Required Qualifications, Training, and Education

Registered Nurse (RN) licensure with a minimum of 3 years of clinical experience

Associated degree or Diploma in Nursing; Bachelor's degree preferred

Experience in health management, care coordination, or telephonic nursing is preferred

Must obtain CCM Certification within 4 years of employment

Ability to hold an unrestricted RN license in the state of residence with multi-state / compact privileges
