Position Summary :
Osceola Medical Center is seeking a compassionate and experienced Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to join our team, with a specific focus on geriatric care . This position offers the opportunity to manage your own outpatient panel while providing collaborative, high-quality care in a rural community setting. The ideal candidate will bring a strong clinical background in geriatrics, excellent communication skills, and a commitment to holistic, patient-centered care.
In addition to outpatient clinic responsibilities, the APC will spend one half-day per week providing care and support to patients in the Transitional Care Unit (TCU) attached to OMC, ensuring smooth transitions, continuity of care, and close coordination with inpatient and skilled nursing staff.
Geriatric Focus :
This position centers on the care of adult and older adult patients, emphasizing chronic disease management, cognitive and functional assessments, preventive care, and age-appropriate interventions.
Qualified candidates will meet one of the following :
- Have a minimum of 1 year of experience in geriatrics , OR
- Hold a Geriatric certification (e.g., ANCC Gerontological NP, PA-CAQ in Geriatrics).
Essential Duties & Responsibilities :
Key Responsibilities
Manage a panel of geriatric and adult primary care patients in the outpatient setting.Provide high-quality care in the TCU one half-day per week.Conduct comprehensive assessments, develop individualized care plans.Diagnose and manage acute and chronic conditions with a geriatric lens.Perform cognitive, functional, and fall-risk assessments.Coordinate care with physicians, nursing, social work, and caregivers.Provide patient and family education on aging-related health concerns.Oversee referrals, diagnostic testing, and follow-up care.Knowledge, Skills, & Abilities :

Graduate of an accredited Nurse Practitioner or Physician Assistant program – requiredLicensed or eligible for licensure in Minnesota and Wisconsin – requiredCurrent DEA license and eligibility for prescription writing privileges – requiredMinimum 1 year of experience in geriatrics OR geriatric-specific certification – requiredMinimum 2 years of APC experience – preferredWhy Join Osceola Medical Center?
