Conduct comprehensive assessments, develop individualized care plans. Diagnose and manage acute and chronic conditions with a geriatric lens. Perform cognitive, functional, and fall-risk assessments. Coordinate care with physicians, nursing, social work, and caregivers. Provide patient and family education on aging-related health concerns. Oversee referrals, diagnostic testing, and follow-up care. In-depth knowledge of geriatric conditions, syndromes, and care approaches Skilled in assessment, diagnostic interpretation, and care planning Proficient in managing polypharmacy and complex medical needs Accurate and efficient documentation in the electronic medical record Compassionate, patient- and family-centered approach Focus on promoting independence, safety, and quality of life for older adults Advocacy for patients across care settings, including the TCU Strong interdisciplinary communication and coordination skills Effective interactions with patients, families, and healthcare teams Respectful, inclusive, and culturally sensitive care delivery Upholds ethical and professional standards Demonstrates flexibility and resilience in a rural care environment Actively participates in continuing education and team development Graduate of an accredited Nurse Practitioner or Physician Assistant program – required Licensed or eligible for licensure in Minnesota and Wisconsin – required Current DEA license and eligibility for prescription writing privileges – required Minimum 1 year of experience in geriatrics OR geriatric-specific certification – required Minimum 2 years of APC experience – preferred Five-day work week with consistent hours and one late evening Dedicated time supporting a collaborative Transitional Care Unit Rural healthcare setting with a tight-knit, supportive team Opportunity to make a lasting impact on the aging population

Position Summary :
Osceola Medical Center is seeking a compassionate and experienced Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to join our team, with a specific focus on geriatric care . This position offers the opportunity to manage your own outpatient panel while providing collaborative, high-quality care in a rural community setting. The ideal candidate will bring a strong clinical background in geriatrics, excellent communication skills, and a commitment to holistic, patient-centered care.
In addition to outpatient clinic responsibilities, the APC will spend one half-day per week providing care and support to patients in the Transitional Care Unit (TCU) attached to OMC, ensuring smooth transitions, continuity of care, and close coordination with inpatient and skilled nursing staff.
Geriatric Focus :
This position centers on the care of adult and older adult patients, emphasizing chronic disease management, cognitive and functional assessments, preventive care, and age-appropriate interventions.
Qualified candidates will meet one of the following :
- Have a minimum of 1 year of experience in geriatrics , OR
- Hold a Geriatric certification (e.g., ANCC Gerontological NP, PA-CAQ in Geriatrics).
