job_description.job_card.job_descriptionHome health setting covering multiple counties, with specific territory assigned during interview Caseload includes geriatric patients and wound care cases Minimum of 23-24 patient visits per week required for full-time hours Day shift schedule from 8 : 00 to 17 : 00, Monday through Friday No weekend or on-call requirements, though on-call may occur at management discretion Electronic charting is completed onsite using Kinnser / Wellsky and cannot be taken home Travel between patient homes is required; mileage reimbursed at IRS standard from first to last patient Free residential parking provided Responsibilities include providing patient-focused home health care and completing all charting in clinic on a provided tablet Qualities valued include punctuality, being a self-starter, and timeliness in documentation First-time travelers are welcome to apply Any color scrubs with closed-toe shoes required COVID-19 vaccination is required, with declination allowed for religious reasons; flu vaccination can be declined with documentation Compliance with all documentation, references, and work history requirements must be met prior to patient care Local candidates within a 50-mile radius are accepted at the same rate Rest and time off (RTO) requests will not be accepted after offer is made Minimum 1 year of recent home health PT experience and 2 years' total clinical experience required Active state PT license required at time of consideration; pending licenses not accepted Current BLS certification required No history of disciplinary actions on license Must meet compliance requirements before starting, which may take 2-3 weeks Ability to travel within assigned and possibly neighboring counties Must submit all time off requests during application process Willingness to work a minimum contract length of 4-6 months, with less experienced clinicians required to commit to 6 months High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts
