Position Summary

The facility is seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join its primary care team in Bronx, NY .

This position offers both full-time and part-time scheduling options, providing flexibility and balance within a supportive, multidisciplinary environment. The Family Nurse Practitioner will deliver comprehensive primary care services to patients of all ages, focusing on prevention, wellness, and management of acute and chronic conditions.

Essential Position Responsibilities

Provide comprehensive primary care services, including preventive, acute, and chronic disease management for patients of all ages.

Conduct physical exams, order and interpret diagnostic tests, and manage treatment plans.

Educate and counsel patients on disease prevention, health maintenance, and lifestyle management.

Collaborate with physicians, specialists, and other members of the healthcare team to ensure coordinated, patient-centered care.

Maintain accurate and timely documentation in the electronic health record (EHR).

Participate in care coordination, case management, and quality improvement initiatives.

Qualifications

Education : Graduate of an accredited Nurse Practitioner program (MSN or DNP).

Licensure & Certification : Active or eligible Family Nurse Practitioner (FNP) license in New York; national certification required.

Experience : Prior experience in primary care or family medicine preferred, but new graduates are encouraged to apply.
