Astrana Health is seeking a dedicated Certified Alcohol and Drug Counselor (CADC) with proven experience in providing therapeutic counseling and case management services to individuals struggling with substance use disorders. The CADC will facilitate quality gap closure for Medicaid members who discharge from the hospital due to a behavioral health or substance use matter by completing telephonic outreach to these members in a timely manner post-discharge. The CADC will complete assessments, deliver evidence-based counseling interventions, develop and manage individualized treatment plans, and coordinate care across multidisciplinary teams to support clients’ recovery journeys.

What You'll Do
Telephonic outreach to members with substance use disorder or behavioral health admit to ER for PCP or behavioral health appointment within 7 days post-discharge and follow-up within 30 days post-discharge.Assist members in scheduling and attending post-discharge follow-up appointment with provider.For members who cannot receive an appointment with their provider, escalate cases to Licensed Behavior Health Counselor to ensure quality gap closure.Conduct comprehensive assessments to evaluate clients’ substance use, mental health, and psychosocial needs.Develop, implement, and update individualized treatment plans incorporating therapy goals and case management strategies.Provide individual counseling using evidence-based modalities such as CBT, Motivational Interviewing, and relapse prevention.Facilitate case management by coordinating referrals to medical, psychiatric, social, and community resources.Monitor client progress and document treatment outcomes accurately in client records and electronic health systems.Collaborate with interdisciplinary teams, including social workers, healthcare providers, and probation officers, to ensure integrated care.Advocate for clients’ needs and support engagement in recovery support services.Educate clients and families about substance use disorders, treatment options, and relapse prevention.Maintain compliance with all regulatory and ethical standards, including confidentiality and HIPAA.Participate in professional development, clinical supervision, and team meetings to enhance skills and service quality.Other duties as assigned.Qualifications
Current certification as a Certified Alcohol and Drug Counselor1+ years of experience providing counseling and case management in substance use disorder treatment1+ year experience as a counselor in a treatment program facility -preferredStrong knowledge of addiction, counseling theories, treatment modalities, and recovery principles.Experience with crisis intervention and co-ocuring mental health disorders.Excellent interpersonal, communication, and documentation skills.Bilingual Spanish preferred.Environmental Job Requirements and Working Conditions
This is a full-time position. Hours are M-F 830 AM - 5 PM.This position operates on a hybrid schedule.
