Job Description :

The State of Nebraska, Department of Health and Human Services is looking for a DD Community Coordinator Specialist at the Nebraska State Office Building in Lincoln.

Examples of Work :

• Coordinate and oversee the delivery of services
• Conduct assessments and eligibility determinations
• Develop plans of services and supports
• Provide appropriate referrals
• Monitor provision of services
• Ensure health, welfare, and safety of those we support
• Ability to travel required. Travel will be based on necessity and could vary from week to week. Potential overnight travel may occur.

Requirements / Qualifications

Minimum Qualifications : Bachelor's degree and professional experience in : education, psychology, social work, sociology, human services, or a related field and experience within a specialized developmental disabilities service system in delivery of habilitation and / or case management services.

Preferred Qualifications : Experience with developing person-centered plans, completing assessments, habilitation, program implementation, team and meeting facilitation, experience in Word, Excel, Outlook, Nfocus and Therap. Experience in services or programs for people with special healthcare needs. Experience working with Assisted living facilities, experience in the behavioral / mental health

Knowledge : the structure of organizations; state, local and federal laws and programs relating to social service programs; available funding sources; human dynamics; learning / teaching techniques.
