- Provide outreach, education, connection, navigation of resources, and support community engagement for residents living independently, to aid them in maintaining independence, self-sufficiency, and well-being.
- Create collaborative relationships with residents, property managers, and community providers to support independent living.
- Develop and grow a network of contacts with service providers and agencies for resident referrals
- Identify the needs of and educate residents on available services, help them to navigate and assist in applying for resources.
- Host or present group workshops for residents using outside providers, or our library of presentation. May be asked to develop presentations.
- With the support of program managers, provide programming as determined by annual community needs assessment.
- Work in conjunction with property management to empower residents to age in place and remain as independent and self-reliant as possible.
- Maintain clearly designated schedule, follow through on appointments, on-going paperwork, and coordination of services.
- Utilize agency database for timely tracking of Resident Engagements (i.e., progress notes, care plans, referrals made, follow-up dates).
- Promote the Transforming Age Culture of Excellence in all areas of responsibility.

- High school diploma or equivalent. Associates' in Social work, public health, or human services preferred.
- An approachable friendly demeanor and a sensitivity to various interpersonal styles.
- Direct experience providing services to low-income households, seniors, and / or adults with disabilities, including the provision of information and referral services preferably in a housing setting.
- Experience in navigating local / county / state / federal systems (DSHS, Medicare / Medicaid, Social Security, mental health, etc.) and community resources with positive outcomes.
- Proven experience in service management including organizing, collaborative problem-solving, de-escalation, and advocating.
- Ability to multitask, fulfill multiple projects with minimal supervision.
- Comfort with working independently while a part of a larger team.
- Proficiency with MS Office suite (Word, Outlook, TEAMS).
- Exceptional written and oral communication and presentation skills in the English language. (Bilingual / Multi-lingual experience preferred)
- Experience working with diverse populations is strongly preferred.
- Completion of a background check and drug screen.
