What You'll Do

As an LPN / LVN, you'll play a vital role in supporting residents and leading our care team. Responsibilities include :

Assessing new and current residents and developing individualized care plans

Monitoring and documenting changes in resident conditions

Communicating regularly with the Director of Health & Wellness, physicians, families, and team members

Providing hands-on care and clinical support to residents

Training and supporting care staff to ensure the highest standards of service

Reviewing Medication Administration Records (MARs) and physician orders for accuracy

Ensuring compliance with all regulatory standards and safety guidelines

What We're Looking For

Current LPN license (required)

At least 1 year of nursing or caregiving experience (memory care / assisted living preferred)

6+ months of supervisory experience

Strong communication and leadership skills

Compassion, patience, and a team-first mindset
