About the Role :

The Family Nurse Practitioner (FNP) at Humble Main, HCC plays a critical role in delivering comprehensive healthcare services to patients across the lifespan, from pediatrics to geriatrics. This position focuses on providing high-quality, evidence-based care in both acute and chronic health conditions, emphasizing patient safety and education. The FNP will collaborate closely with interdisciplinary teams to develop and implement individualized care plans, ensuring optimal patient outcomes. Additionally, the role involves managing complex cases including chemotherapy administration, pain management, and urgent care interventions. Ultimately, the FNP serves as a primary healthcare provider dedicated to improving the health and well-being of the community through compassionate and skilled nursing practice.

Minimum Qualifications :

  • Current and unrestricted Family Nurse Practitioner (FNP) license.
  • Master’s or Doctoral degree in Nursing from an accredited program.
  • Certification as a Family Nurse Practitioner by a recognized certifying body (e.g., ANCC or AANP).
  • Minimum of 2 years clinical experience in family practice or related healthcare settings.
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.

Preferred Qualifications :

  • Experience in chemotherapy administration and oncology nursing.
  • Background in critical care or urgent care settings.
  • Additional certifications in pain management or patient safety.
  • Familiarity with electronic health record (EHR) systems and healthcare informatics.
  • Strong communication and interdisciplinary collaboration skills.
  • Responsibilities :

  • Conduct comprehensive health assessments and physical examinations for patients of all ages.
  • Develop, implement, and manage individualized treatment plans, including prescribing medications and therapies.
  • Provide patient education on disease prevention, management, and health promotion to enhance patient understanding and compliance.
  • Administer and monitor chemotherapy treatments and manage associated side effects in collaboration with oncology teams.
  • Deliver urgent care services, including triage and stabilization of acute conditions, ensuring timely and effective interventions.
  • Monitor patient safety protocols rigorously to minimize risks and ensure compliance with healthcare regulations.
  • Collaborate with physicians, specialists, and other healthcare professionals to coordinate multidisciplinary care.
  • Document patient encounters accurately and maintain up-to-date medical records in accordance with legal and ethical standards.
  • Skills :

    The required skills such as patient education, chemotherapy administration, and pain management are integral to daily clinical practice, enabling the FNP to provide holistic and specialized care tailored to individual patient needs.
