JOB PURPOSE :
As part of PruittHealth's (PH) top initiatives, a new division of Personal Care Services (PCS), PH Family First, is being piloted metro Atlanta market. PH Family First is looking to hire a PCA to join a winning team, to assist the patient and caregiver with personal hygiene needs to increase the level of comfort to the patient.
KEY RESPONSIBILITIES :
- Provide optimal comfort care, dignity and privacy for the patient
- Obtain vital signs of patient(s) assigned
- Follow procedures for handling medical emergencies or other incidents that affect the delivery of services
- Report patient progress and problems to supervisory personnel
- Observe and report any changes in condition, including skin integrity
- Provide comfort care, companion, shopping / errands, meal prepping and related tasks, housekeeping, appointment scheduling, medication reminders services
- Assist with patient transfer, ambulation, and exercise
- Provide personal hygiene to patient(s), including bathing, shaving, oral hygiene, skin care, dressing, toileting, and hair care
- Maintain a safe, clean and comfortable environment for the patient
- Coordinate care with agency RN Supervisor
- Maintain accurate and current records of visits
- Perform all other duties as assigned
KNOWLEDGE, SKILLS, ABILITIES :
Attend a minimum of 12 hours of annual in-service education and provide documentation to the agencyAttend team meetings and staff development in-services and trainingsCommunicate well with patient and family members. Provide a warm and friendly greeting and an approachable attitude to families, visitors, and patients. Respond to expressed concerns with a helpful, caring demeanor. Answer questions in a professional mannerRespond to change productively, and handle additional projects as assignedParticipate in agency surveys and any subsequently required reportsAttend and participate in continuing educational programs to keep abreast of changes in the fieldMaintain current license / certification as requiredHonor patient's rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaintsComply with corporate compliance programReport job-related functions / tasks that involve occupational hazards, including exposure to blood and body fluids, as necessaryFollow established safety regulations, including fire protection and prevention, smoking regulations, infection control, etcFollow established safety procedures when performing tasks and / or working with equipmentAbility to use Matrix Care documenting system, after receiving trainingMINIMUM EDUCATION REQUIRED :
Certificate of completion of state recognized Home Care Aide training program and supervised clinical practiceMINIMUM EXPERIENCE REQUIRED :
One (1) year of experience working with adults in a healthcare settingMINIMUM LICENSURE / CERTIFICATION REQUIRED BY LAW :
Current registration in State Nurse Aide Registry as a Certified Nurse AideCurrent CPR certificationValid driver's license and automobile liability insuranceADDITIONAL QUALIFICATIONS : (Preferred qualifications)
