This position includes skilled nursing services and supervision of hospice aide staff to ensure personal patient care is delivered effectively.\n\nKey Responsibilities : \n
- Take primary responsibility for a patient / family caseload, conducting assessments, planning, implementation, and evaluation of care needs during on-call hours in coordination with the triage team.\n
- Communicate and report on-call activities to clinical leadership and Branch RN Case Managers at the end of your call shift.\n
- Maintain comprehensive care coordination notes for visit documentation and other clinical information.\n
- Assist in providing or arranging delivery of medications and supplies needed outside regular business hours, including weekends and holidays.\n
- Participate in interdisciplinary team meetings as required during your on-call period to engage in discussions and evaluations of patient / family needs.\n
- Admit patients eligible for hospice services under the physician's orders and in collaboration with the triage team.\n
- Report patient status and needs for additional services to clinical leadership and attending physicians.\n
- Develop individualized patient care plans that address specific needs identified through mutual agreement with patients during their care.\n
- Ensure the integration of the 4Ms framework (What Matters to the patient, Medications, Mentation, and Mobility) to provide Age-Friendly Care.\n\nSkills and Experience Required : \n
- Current RN license in the state of practice or in accordance with the Nurse Licensure Compact (NLC).\n
- Valid driver's license with access to a reliable vehicle and proof of auto insurance.\n
- Minimum of two years' recent experience as a professional nurse.\n
- Ability to meet physical demands associated with nursing, including standing, bending, lifting, and less than 50 pounds of physical exertion.\n
- Proficient in working independently and making sound nursing judgments in a field setting.\n
- Capable of assessing patient needs and creating tailored patient care plans.\n
- Understanding of the physical, psychosocial, and spiritual needs of terminally ill patients and their caregivers.\n
- Strong written and verbal communication skills.\n\nPreferred Knowledge, Skills, and Experience : \n
- Two years' experience within hospice care.\n
- Certification in Hospice and Palliative Nursing.\n
- Experience with Electronic Medical Records systems.\n\nThe role is an on-call position intended to provide critical support to our hospice services, ensuring that patients receive the care they need, when they need it.
