RVNAhealth is seeking a compassionate and experienced Hospice Clinical Manager to lead and support our Hospice nursing and coordination teams. This leadership role is pivotal in ensuring exceptional, person-centered end-of-life care. The Hospice Clinical Manager will be responsible for day-to-day clinical operations, team supervision, regulatory compliance, and workflow management-all while fostering a culture of accountability, empathy, and clinical excellence.

This position will support the Southern Territory of Fairfield County (including Norwalk, Stamford, Greenwich and surrounding towns).

Key Responsibilities

Oversee daily operations of hospice clinical services, ensuring timely and compliant care delivery.

Supervise and mentor RN, LPN, and Coordinator staff , including performance evaluations and professional development.

Manage workflow tasks in HomeCare HomeBase (HCHB) , ensuring timely completion of clinical orders to prevent delays in patient care.

Provide real-time feedback to clinicians on documentation, clinical effectiveness, and symptom management.

Lead interdisciplinary team (IDT) meetings , ensuring care plans meet all hospice and regulatory requirements.

Conduct joint field visits and provide ongoing training and clinical support.

Collaborate with the Manager of Support Services and QAPI team to maintain high standards of care and continuous improvement.

Participate in the Administrator On-Call rotation as needed.

Assist in staffing strategy, hiring decisions, onboarding, and retention efforts.

Contribute to policy review, budgeting, program planning , and strategic initiatives related to clinical services.

Qualifications

Master of Science in Nursing (MSN) with at least 1 year of full-time clinical hospice experience, or

Bachelor of Science in Nursing (BSN) with at least 3 years of full-time clinical experience, including 1 year in hospice.

Current Registered Nurse (RN) license in the State of Connecticut.

Proficiency with electronic medical records; experience with HomeCare HomeBase (HCHB) preferred.

Strong critical thinking, workflow optimization, and multitasking skills.

Deep knowledge of Medicare Conditions of Participation , State of CT Licensure , CHAP Standards , and HIPAA .

Demonstrated ability to lead, coach, and inspire a multidisciplinary team.

Compensation :

The pay range listed represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
