Job Description :

• Provide skilled PT services to adult and geriatric patients in their homes
• Perform evaluations, develop care plans, and deliver therapeutic interventions
• Accurately document services using EMR system : Wellsky (Kinnser)
• Educate patients and caregivers to improve safety, mobility, and function
• Collaborate with nurses and therapists for patient-centered outcomes

Requirements :

• Active Texas Physical Therapist License
• EMR Experience – Wellsky / Kinnser is a Plus
• Home Health Experience is a plus
• Current CPR Certification
• Reliable Transportation, Valid Driver’s License, and Auto Insurance
