About the Position :

The Home Health Registered Nurse will serve as the Case Manager coordinating comprehensive home care for patients. You will collaborate with various medical professionals and deliver professional care directly to patients in their homes. Responsibilities include conducting admission visits, assessing patient eligibility for home care services, and crafting individualized care plans.
