Licensed Practical Nurse - LTC

Summary Summary Under the supervision of the Registered Nurse / Director of Nursing, the Licensed Practical Nurse utilizes a general understanding of the principles of nursing and basic physical assessment skills in the development of and implementation of individualized nursing care plans to ensure that the needs of the residents are met. Attends to the daily operations of the unit on a per shift, unit level, and assumes responsibilities of a leadership role as needed. Responsibilities In a team relationship, conducts ongoing reassessment and gathers data relevant to the individual needs of the patient or family. Performs ongoing focused systems measurements, as appropriate to specialty; obtains patient history, including objective and subjective data. Collects data for the admission database per policy, in accordance with specific unit standards. Observes patient's clinical condition; reviews diagnostic information; recognizes implications of abnormal diagnostic results; identifies problems typically associated with a diagnosis or treatment; reports pertinent information to appropriate personnel for further directions. Performs individual patient care according to established plan of care and standards of practice. Identifies and documents patient / family / significant other spiritual and cultural needs.
