Position Summary
The Youth and Young Adult (YYA) Case Manager supports individuals ages 18–24 in the agency’s Youth and Young Adult Transition in Place (YYA TIP) program. As a member of the Housing & Shelter team, the Case Manager provides comprehensive case management services—including housing navigation, income development, and connections to behavioral health and medical care—to young adults transitioning out of homelessness. This role is dedicated to promoting housing stability and self-sufficiency, with the ultimate goal of ensuring that homelessness is brief, rare, and nonrecurring.

Qualifications
Our ideal candidate has a High School Diploma or GED; Bachelor’s Degree preferred. Some experience working with people experiencing homelessness, veterans, people with substance use disorders, people with re-entry backgrounds, and / or other relevant social service populations, preferred. A Valid Ohio Driver’s license and proof of insurance, required
