GUIDE Program Coordinator (Remote, US)

We are seeking a mission-driven GUIDE Program Coordinator to join a growing healthcare initiative that supports families caring for loved ones with dementia. In this role, you'll serve as the vital bridge between enrollment and engagement guiding patients and caregivers through each step of the program with empathy, precision, and structure. What Youll Do

  • Guide patients and caregivers through onboarding after enrollment.
  • Monitor completion of dementia screenings and health assessments.
  • Coordinate device (tablet) shipments and provide orientation to the GUIDE program.
  • Schedule and track neurology evaluations with clinical partners.
  • Review documentation and prepare applications for CMS submission.
  • Track participant progress and share status updates with internal teams.
  • Ensure a smooth handoff to Engagement Specialists and Coaches.
  • Collaborate on software workflow improvements and process evolution.
  • Contribute to continuous improvement of program operations.
  • Who You Are

  • Background : Experience in operational, patient-facing, or practice management roles within healthcare, health tech, or non-profit settings.
  • Mission-Driven : Strong personal or professional connection to dementia and caregiving. This empathy and passion for supporting families is a must-have.
  • Organized & Detail-Oriented : Capable of managing 4060+ patient cases simultaneously with strong follow-through.
  • Strong Communicator : Able to connect with caregivers in a compassionate, clear, and patient-centered manner.
  • Adaptable : Comfortable with evolving processes and collaborating across teams.
  • Bonus Skills : Familiarity with CMS, dementia programs, or GUIDE requirements is an advantage.
