Speech Language Pathologist - Mind&Mobility

Location : Medicare Part B Visits- Wellington Area- PRN

Job Type : Independent Contractor (1099)

Pay : $75 / visit

Position Summary :

We are seeking a Speech-Language Pathologist to provide in-home care for Medicare Part B patients. In this role, you will assess and treat speech, language, cognitive-communication, and swallowing disorders, creating individualized therapy plans to help patients improve communication, swallowing, and overall quality of life.

Key Responsibilities :

  • Assess patients in their homes and create individualized therapy plans.
  • Provide skilled interventions for speech, language, cognition, and swallowing.
  • Educate patients and caregivers on strategies to improve communication and daily functioning.
  • Document progress and ensure compliance with Medicare Part B regulations.
  • Collaborate with the home health / therapy care team to coordinate patient care.
  • Adjust treatments based on patient progress and needs.

Qualifications :

  • Active Speech-Language Pathology (SLP) license in the state of practice.
  • Certificate of Clinical Competence (CCC-SLP) required.
  • Experience in home health or outpatient therapy preferred.
  • Knowledge of Medicare Part B documentation and regulations.
  • Strong assessment, treatment planning, and patient education skills.
  • Excellent communication and interpersonal abilities.
  • Reliable transportation and a valid driver's license.
  • Comfortable working independently and managing a flexible, home-based schedule.
  • Proficiency with electronic medical records (EMR) and basic computer skills.
  • CPR and other relevant certifications as required.
