• Associates degree, preferred\n
• Registered Nurse license required in state(s) practicing\n
• Current and valid CPR certification, preferred – required in Georgia\n
• Minimum one year of experience, preferred – required in California\n
• Previous experience in hospice, home health, general medical surgical or patient population specific (i.e. : geriatrics,\npediatric, etc.) nursing\n\nResponsibilities : \n
• Conducts comprehensive nursing assessments and develops individualized care plans in coordination with the interdisciplinary team (IDG), addressing physical, emotional, social, and spiritual needs\n
• Maintains timely and accurate documentation of care, interventions, and patient status updates, ensuring compliance with internal protocols and regulatory requirements\n
• Coordinates care activities including visit scheduling, medication management, physician collaboration, and interdisciplinary communication to ensure continuity and quality of care\n
• Provides education and support to patients and caregivers, and responds to urgent needs during on-call or admissions assignments\n
• Supervises LPNs, LVNs, and hospice aides as needed, and participates in IDG meetings, patient conferences, and ongoing clinical education\n
• Supports pre-hospice services by evaluating patient status via phone, telehealth, or in-person visits and identifying needs for additional care or services\n\nOur Investment in You\n\nCaring for others starts with caring for you.
