The Family Practice - Without OB Physician provides comprehensive primary care services including diagnosis, treatment, and preventive care within a Patient Centered Medical Home (PCMH) framework. Position Overview :

The Physician's overall responsibility is to provide a full scope of primary care services which fall under their field of training. The Physician is to administer coordinated and comprehensive professional health care services that include, but are not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance for the patients at SMF. In addition, the physician is part of a Patient Centered Medial Home (PCMH) team and is expected to be familiar with the philosophy 1 goals and objectives of the PCMH.
