Your Responsibilities as a Home Health Nurse :

Deliver compassionate home-based care to patients recovering from illnesses, injuries, or chronic diseases

Collaborate with a team of physicians and oversee LPNs, CNAs, and aides in providing exceptional patient care

Conduct patient assessments, document progress, and communicate changes to physicians

Educate patients and their families on care plans, medications, and home safety

Ensure patient goals are met and coordinate discharge from services

What We’re Looking For :

Active, unrestricted Registered Nurse (RN) license in the State of Michigan – BSN not required

At least one (1) year of clinical experience as a Registered Nurse

Current CPR Certification

Reliable transportation with a valid state driver's license and current car insurance
