Job Title : Post-Acute Care Physician / Medical Director (Full-Time or Part-Time)
Location & Environment
You will provide medical leadership and direct patient care in post-acute, long-term care, rehabilitation, and assisted living settings. You'll collaborate with nursing and therapy teams across facilities to deliver high-quality, patient-centered care.
Role Overview
- Serve as Medical Director or Attending Physician overseeing resident care, regulatory compliance, and clinical standards across facility locations.
- Deliver evidence-based primary care, diagnosis, and ongoing management of complex medical conditions in long-term care populations.
- Act as a clinical mentor : guide and support nurse practitioners, physician assistants, nursing staff, and care teams to optimize care delivery.
- Ensure compliance with all regulatory, quality, and safety standards in alignment with CMS, state health agencies, and facility policies.
