Your Responsibilities :

• Evaluate Patient Needs : Assess functional status and physical therapy requirements, collaborating with physicians to develop tailored therapy plans.
• Deliver Quality Care : Provide services within your scope of practice, ensuring coordination with the healthcare team.
• Monitor Progress : Observe and report patient responses to treatment, keeping the nurse supervisor and physician informed.
• Implement Treatment : Use evidence-based techniques to relieve pain and restore function, helping patients achieve their maximum potential.
• Educate and Inform : Guide patients, families, and healthcare team members on care plans, functional limitations, and progress toward goals.
• Documentation : Complete progress notes and evaluations in accordance with LHC policy and state guidelines.
• Participate in Team Efforts : Engage in staff conferences and committees, contributing to a collaborative work environment.
• Supervise and Support : Oversee home health aides and Physical Therapist Assistants as per policy and regulations.
• Weekend Rotation : Participate in weekend rotations to provide essential services as needed.
• Discharge Summaries : Prepare written summaries of patient conditions every 60 days or as required.
• Continuous Learning : Stay updated with current standards of care and maintain your licensure through ongoing education.
• And other duties as assigned.
