RN Nurse Case Manager

A company is looking for a Nurse Case Manager, who will support patients through assessment, planning, facilitation, and advocacy in a remote setting.

Key Responsibilities

Utilize assessment, planning, facilitation, and advocacy to coordinate patient care

Ensure services align with benefit plans and meet patient health needs

Collaborate with members, providers, and care teams to deliver coordinated care

Required Qualifications

High School Diploma or equivalent; Bachelor's Degree preferred

RN with compact unrestricted active license

2-3 years of clinical practice experience in healthcare settings

2 years of experience in the healthcare and / or managed care industry

Case Management experience preferred
