Pay range : $31.40 - $37.44 / hr plus mileage reimbursement or company car program

Key Responsibilities :

Deliver physical therapy services at patients’ homes under PT supervision

Coordinate interdisciplinary patient care assessments

p] : mb-2 [&>

p] : my-0">

Document treatments and patient progress accurately and timely

p] : pt-0 [&>

p] : my-0">

Educate patients and caregivers to advance therapeutic goals

Must Have + Certifications :

Active licensed PTA in Minnesota in good standing

p] : pt-0 [&>

p] : mb-2 [&>

p] : my-0">

Preferred 2+ years of PTA experience

p] : pt-0 [&>

Valid driver’s license and proof of auto insurance

Current CPR / BLS certification
