Home Care RN – Nights (On-Call)

Location : Monmouth County

The VNA Health Group is seeking a compassionate and dedicated Home Care Registered Nurse (RN) to join our team. This vital role provides after-hours care and support to patients and families, ensuring exceptional clinical service and comfort in the home setting.

Responsibilities :

• Patient Care : Deliver high-quality nursing care during after-hours, addressing physical, emotional, and educational needs.
• Emergency Response : Respond promptly to calls, conduct home visits as needed, and manage urgent patient needs.
• Symptom Management : Assess and manage pain and other symptoms to promote patient comfort.
• Family Support : Provide guidance and reassurance to families during challenging situations.
• Collaboration : Communicate effectively with physicians, case managers, and interdisciplinary team members.
• Documentation : Maintain accurate, timely records of all patient care and communications.
• Adhere to Best Practices : Uphold VNA Health Group’s standards for patient-centered care and quality outcomes.

Qualifications :

Working Conditions / Physical Demand :

Combination of indoor and outdoor work; includes patient contact, use of clinical equipment, and electronic documentation.

Compensation :

In compliance with New Jersey’s Pay Transparency Act, the total compensation range for this position is $60,000 – $110,000 , which reflects base pay, overtime, bonuses, and other incentives. Actual compensation will depend on experience, skills, and internal equity.
