The role focuses on improving patient outcomes, satisfaction, and cost efficiency.

Requirements :

BSN and active Michigan RN license

Experience with AllScripts or GEMS / Sunrise systems

3+ years of RN clinical experience

5+ years acute inpatient case management experience

AHA BLS certification

CarePort experience required

Knowledge of discharge planning, utilization management, and managed care

Strong communication, organizational, and problem-solving skills

Setting : Acute inpatient

Must be adaptable and comfortable working directly with patients on the floor
