Position Overview As a Home Health Case Manager, you will play a key role in collaborating with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within Boise, ID and surrounding regions to deliver in-home care from admission to discharge. Your work will ensure that each patient receives the highest standard of care, aligned with their needs and the care plan. • Full-time Schedule : Monday to Friday, 8 : 30 AM - 5 : 00 PM, with rotating evening on-call and weekend shifts
• Patient Population : Adults
• Manageable Caseloads : Daily visit requirements are below the state and national average
• Compensation : Paid hourly which includes drive time and mileage reimbursement for traveling within Boise and it's surrounding regions
• Additional Pay : On-call pay and applicable differential pay

• Education : ASN or BSN in Nursing
• Experience : 2 years of RN experience; Home Health or Acute Care experience preferred
• Licenses / Certifications : Current RN license in the state of Idaho; Basic Life Support (BLS) certification
