License : Valid state license required.
Certifications : License verification is necessary.
Must-Have :
- SNF experience preferred.
- Covid test required 72 hours prior to start date, with results issued 48 hours prior.
Description :

Documentation includes a prescreen form, resume, and license verification.
Candidates will be declined if these items are not completed and attached.
Candidates must provide references (three required).
A county criminal background check (7 years for all counties lived and worked) is needed.
A federal criminal background check (7 years or more) is required.
A multi-state criminal search (7 years or more) is necessary.
A nationwide sex offender search (7 years or more) must be completed.
HHS / OIG and GSA searches are required.
Social Security number trace must be performed.
A 1 step PPD or Quantiferon proof of a positive PPD and chest X-ray is required.
A SAM search must be completed.
A negative drug screen result is mandatory.
A Louisiana state background check is required.
