Licensed Practical Nurse – Home Health
Location : Vancouver, WA
Schedule : Full Time, 1.0 FTE, Day shift
Salary Range : $28.36 – $42.03 per hour (dependent on education, training, experience, and collective bargaining agreement terms)

We are currently seeking a Licensed Practical Nurse (LPN) – Home Health to join our Vancouver team. This role is an excellent opportunity for an LPN who values teamwork, professional growth, and making a meaningful difference in the lives of patients and families.
What you’ll do :
Provide direct nursing care under the supervision of an RN, following physician orders and established care plans.
Administer prescribed medications and treatments, and monitor patient responses.
Document all care accurately in accordance with department standards.
Assist in the development and evaluation of patient-specific care plans.
Perform wound care, dressing changes, vital sign monitoring, patient education, and other direct care duties.
Collaborate with the healthcare team to ensure safe, timely, and patient-centered care.
Support daily operations by transporting supplies, equipment, and specimens as needed.
Maintain professional effectiveness through ongoing education and development.
Qualifications :
Graduate of an accredited State Board of Nursing LPN program.
Current Washington State LPN license.
Current Basic Life Support (BLS) certification.
Valid driver’s license, auto insurance, and reliable transportation.
Minimum 1 year of LPN clinical experience preferred.
Knowledge of age-specific principles of growth, development, and life stages.
