Reason for Coverage : LOA and departure Practice Setting : Hospital Based-all inpt-OR ICU / Rounding of patients in ER and floors Schedule (Shifts and Hours) : M-F 6 : 45AM-5PM Call Schedule : nights and weekends- TBD- approx. 7-8 calls per month Call Ratio : 3 providers taking call Average Number of Cases Per Day : 2 cases per day Trauma Level / Designation : Level One Annual ER Volume : 85,000 per year Number of Operating Rooms : 3 Documentation System / EMR : EPIC Description of Ancillary Staff : Full complement Adult and Geriatric Certification Requirement : BLS and ACLS Procedures Required : Descending Aortic Aneurysms, CABG, Off-Pump CABG, Arterial Revascularization -Using Internal Mammary Arteries, Radial Artery and Gastroepiploic Artery, Redo Coronary Revascularization, Tube Thoracostomy, Pericardial Window, Thoracic aortic aneurysms, Heart Valve Repair / Replacement Surgery, CoreValve System, Mitral Valve Repair SUBMISSION REQUIREMENTS - Board Certified- Required BLS and ACLS- Required CT license- Highly preferred- will give preference to providers with CT license.
