The LPN will play a key role in supporting residents’ day-to-day needs, administering medication, and collaborating with RNs, CNAs, physicians, and rehab staff to ensure the best possible outcomes. Key Responsibilities Administer medications and treatments as prescribed Monitor resident conditions and report changes to RN / MD Maintain accurate and timely documentation Provide hands-on care and emotional support to residents Supervise and mentor CNA staff Collaborate with interdisciplinary care team to implement care plans Ensure compliance with DOH, CMS, and facility policies Interested in hearing more?
