Position Summary
The Advanced Practice Provider (APP) is a key member of our outpatient mental health team, responsible for providing comprehensive psychiatric care to patients with diverse needs. This role includes conducting evaluations, managing medications, and collaborating with a multidisciplinary team to deliver patient-centered, evidence-based care in a supportive environment.
Qualifications
- Education : Master's or Doctoral degree in Nursing from an accredited program.
- Licensure : Current licensure as a Psychiatric Mental Health Nurse Practitioner (PMHNP or PMHNP-BC).
- Experience : Minimum 2 years of clinical psychiatric experience (5+ years preferred).
- Knowledge : Familiarity with The Joint Commission, OSHA, CMS, DHS, and state / federal regulatory standards.
- Additional Requirements : Must complete TB testing, drug screening, and background check upon hire.
Primary Responsibilities
Clinical Care
Conduct psychiatric evaluations, including history, mental status exams, and diagnostic assessments.Formulate accurate psychiatric diagnoses and develop individualized treatment plans.Manage psychotropic medication regimens, including initiation, titration, and monitoring.Provide crisis intervention and risk assessments for patients in acute distress.Collaboration
Partner with psychiatrists, therapists, social workers, and other providers to deliver cohesive care.Participate in case consultations and team meetings to review patient progress.Refer patients to higher levels of care or community resources when appropriate.Patient Education & Advocacy
Educate patients and families on psychiatric conditions, treatment options, and self-care strategies.Advocate for patient needs within the healthcare system and community support.Documentation & Compliance
Maintain accurate and timely records in the electronic health record (EHR) system.Ensure compliance with clinic policies, legal regulations, and ethical standards.Monitor and adhere to DEA and state guidelines regarding controlled substance prescribing.Professional Development
