Job purpose :
Do you want to make a difference in the lives of others in your community? Our Designated Coordinator position works with individuals to provide home based care and services on their behalf, as well as contributes to the company and management team with the goal of improving and maintaining high-quality performance standards and reputations within the communities we serve. This position will be the lead for our Individual Home Supports program and a company car may be provided for use.
Duties and responsibilities :
- Ability to write, understand, and implement a variety of Community Support Plans and Individual Service Plans
- Train and manage assigned staff members according to recipient's needs and goals
- Maintain compliance with federal and state standards and laws
- Coordinate meetings between staff, individuals, case managers, and guardians
- Gather current and accurate information in order to support the individual's needs and goals
- Accurately complete and update necessary paperwork
- Ability to track and chart data on a daily basis in compliance with company expectations
- Communicate frequently with guardians, management, and caseworkers
- Ability and willingness to fill in Direct Support Professional roles as needed
- Uphold confidentiality and privacy of individuals and staff members
- Maintain an average of 40% (16 hours) client contact time and 60% (24 hours) administrative time average weekly
Qualifications :
Qualified applicants must be at least 18 years of age and possess a High School Diploma, GED, or equivalent. Other qualifications include :
4+ years of experience working with individuals of varying needs and diagnosesAbility to pass a background check with the MN Department of Human ServicesKnowledge of federal and state healthcare laws and regulationsStrong 245D FamiliarityTraining and management experienceWillingness to travel to and from surrounding area communities as neededVehicle, driver’s license, and valid auto insurance per the State of MinnesotaClear driving record (No DUI or DWI convictions in the past 5 years)Fluently read, understand, speak, and write the English LanguageDynamic communication skills, working techniques, and ability to multitaskPreferred Qualifications :
Associate’s degree, or higher, in social work, human services, or similar studiesCurrent CPR and First Aid CertificationMultilingual - fluent, ability to translate if necessaryBenefits :
