PLACE OF PERFORMANCE

The Contractor shall provide 10 Licensed Practical Nurses (LPNs) to cover for Nursing vacancies or lack of available full-time employees while recruitment and hiring is in process, in accordance with the terms and conditions outlined herein, to the Charleston. The contract staff must be local to the main facility and outlying CBOC’s (VA Clinics); there will not be any travel contracts . Charleston facilities include, but are not limited to :

    Off-duty hours are those hours in which the Contractor's employees are not at the Medical Center in performance of the required services.

    SERVICES PROVIDED

    Licensed Practical Nurse (LPN). Contractor will ensure that Contract LPN provides skilled nursing care to patients in Acute Care, Primary Care and Specialty Clinics in Charleston and VA Clinics (CBOC’s). He / she may perform those approved Medical Center procedures that are described in the Licensed Practical Nurse Act, Vocational Nurse VA Qualification Standards. The LPN demonstrates increasingly more independence in his / her management of patient care and demonstrates competence in caring for adult psychiatric, medical, and geriatric patients. Under the supervision of the nurse manager, the LPN works closely with others as needed in the Specialty Clinics and Primary Care setting throughout the Ralph H. Johnson VAMC and VA Clinics (CBOC’s). The LPN functions both dependently and independently with guidance and direction from the Nurse Manager / designee.

    Licensed Practical Nurse (LPN)

Johnson VAMC, and surrounding Community-Based Outpatient Clinics. He / she may perform those approved Medical Center procedures that are described in the Licensed Practical Nurse Act, Vocational Nurse VA Qualification Standards. The Contract LPN demonstrates increasingly more independence in his / her management of patient care. The contract LPN demonstrates competence in caring for adult psychiatric, medical, and geriatric patients.

    Under the supervision of the nurse manager, the LPN works closely with others as needed in the Acute Care Medical / Surgical areas and Primary Care setting throughout the CHARLESTON VA and surrounding Community-Based Outpatient Clinics. The LPN functions both dependently and independently with guidance and direction from the Nurse Manager / designee. LPN must have active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

  • Contractor nurses may be utilized as needed in the Acute Care Medical / Surgical areas, in ambulatory / outpatient care clinics, and outpatient lab.
  • Knowledge and skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and / or
  • intravenous medications as permitted by approved local facility
  • policies and procedures. Observation and documentation will include patient's response to medication administered and the reporting of any noted change in patient's condition to RN or MD / DO.
  • Demonstrated ability to serve as a responsible member of the nursing team and interact [in an appropriate manner] with patients, family members, professional and [other] supportive personnel [involved in the delivery of] patient care, incorporating acceptable, established customer service standards into practice.
  • Education requirements : required to be a graduate of a state approved school of practical / vocational nursing.
  • QUALIFICATIONS

    The minimum requirement for all LPNs assigned to the CHARLESTON VA shall include a current

    unrestricted LPN license from any state (United States), and a current Cardiopulmonary Resuscitation (CPR) / Basic Life Support (BLS) certificate accredited by the American Heart Association.

  • Med / Surg- minimum 2 years current experience (must have worked within the last six months) in a medical / surgical unit. Agency licensed nurses assigned to Charleston VA Med / Surg unit can expect to work 12-hour shifts, with alternating weekend shifts preferred.
  • Long Term Care / Nursing Home - minimum 2 years current experience and, (must have worked within the last six months) in a long-term care setting. Agency licensed nurses assigned to Charleston VA Long Term Care unit can expect to work 12-hour shifts, with alternating weekend shifts preferred.
  • Psychiatric / Mental Health - Minimum 2 years current experience (must have worked within the last six months) experience in an inpatient psychiatric unit. Agency licensed nurses assigned to
  • Charleston VA Mental Health Psychiatric unit can expect to work 12-hour shifts, with alternating weekend shifts preferred.
  • Primary Care / Specialty Clinics - Minimum 2 years current experience and (must have worked within the last six months) in a primary care setting. Agency licensed nurses assigned to Charleston VA Primary Care / Specialty Clinics unit can expect to work 8-10-hour shifts.
  • May be required to work with COVID 19 positive patients and Patients Under Investigation (PUI), for COVID 19
  • ADDITIONAL REQUIRED QUALIFICATIONS

    In addition to the immunizations mentioned the vendor will also include, at their expense, in the Orientation file for each employee, the following information :

  • South Carolina State Criminal Background Check dated within ninety (90) days of submission
  • A physical dated within ninety (90) days of submission of employee for Contract performance.
  • A current AHA accredited CPR card with an expiration date beyond the last day of orientation (though it is noted that Contractor personnel will be required to maintain CPR certification to perform on the Contract, the initial / orientation requirement is a CPR card with an expiration date beyond the end of orientation).
  • Results of a Contractor developed Facility Staffing Guide Exam completed within (one) 1 year of submission.
  • Results of an OSHA Healthcare Safety Exam dated within (one) 1 year of submission.
  • Current Resume for RNs, LPNs, and Certified Nurse Assistants
  • Occupational Health Testing

    The Contractor must provide documentation of compliance for all personnel performing services under this agreement :

  • Documentation of 2-step PPD Testing
  • Documentation of current Tetanus Toxoid
  • Immunity to Rubeola, rubella, varicella
  • Documentation of current Hepatitis B Vaccination
  • Documentation of current COVID-19 Vaccination
  • Based on the COVID-19 vaccine mandate to include Hybrid Title 38, and Title 5 Department of Veterans Affairs (VA) healthcare personnel-such as RNs, LPNs, and NAs, which includes volunteers and contractors, effectively this means that any Veterans Health Administration (VHA) employee, volunteer or contractor who works in VHA facilities, visits VHA facilities or provides direct care to those we serve are required to be vaccinated. The contractor employees must provide a copy of their and or provide exemptions (religious or medical) approved by the contractor not government.

    The Contractor Health Requirements Checklist shall be completed for each Contract employee providing services under this Contract and forwarded to the COR prior to work start. This and additional information will be incorporated into the VA Orientation file that is prepared for each Contract employee prior to starting work at the CHARLESTON. This Orientation file changes from time to time to reflect new standards or improved efficiencies within the CHARLESTON. Current requirements will be provided by the COR.

  • A regular annual re-testing program for TB shall be established and maintained by the Contractor. Contractor shall provide evidence of retests to COR within ten (10) calendar days of retest.
  • Hepatitis B Vaccination – Contractor and the Contractor employees shall consider the need for Hepatitis B vaccine and vaccination series within ten (10) days of Contract employment and follow-up if there has been an exposure incident. VA shall not provide the vaccine or vaccination series.
  • VA Form 10-5549c, Information About Hepatitis B Vaccine (Recombinant), Consent Form, or VA Form 10-549d, Hepatitis B Vaccine Declination (Mandatory) shall be submitted to the COR within thirty (30) days of waiver, or completion of the first in the series of the Hepatitis B vaccine, and at completion of the vaccination series.
