Job description : Overview

We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to patient care and demonstrate proficiency in various nursing practices. As an LPN, you will play a vital role in delivering high-quality healthcare services, supporting patients in their recovery, and collaborating with other healthcare professionals to ensure optimal outcomes.

Duties

Provide direct patient care, including administering medications, performing wound care, and assisting with daily living activities.

Conduct thorough patient assessments and monitor vital signs to ensure ongoing health and safety.

Collaborate with registered nurses and physicians to develop and implement individualized care plans tailored to each patient's needs.

Maintain accurate medical documentation and records in compliance with HIPAA regulations and organizational policies.

Assist in triage processes, prioritizing patient needs based on severity of conditions.

Educate patients and their families on health management, medication administration, and preventive care strategies.

Participate in infection control measures and maintain a clean, safe environment for patients.

Support patients with developmental disabilities, Alzheimer's care, dementia care, and other specialized needs.
