Collaborate with clinical and operational leaders on workflows for care management, preventive care, and chronic disease monitoring. Analyze population health data and metrics to support targeted care strategies. Develop dashboards and reports to monitor performance and quality indicators. Assist with upgrades, testing, and change management. Stay current with Epic upgrades, functionality, and best practices. Epic Healthy Planet certification (minimum 2 years) Hands-on experience configuring, building, and maintaining Healthy Planet module (2 years) Experience collaborating with clinical / operational teams (2 years) Strong background in population health data analysis and reporting (2 years) Prior experience in a hospital and / or clinical setting
