Position Overview :

As a Care Coordinator, you will build trusting relationships with the individuals you serve while playing a key role in supporting their overall well-being and independence. Your responsibilities will include conducting assessments, developing personalized care plans, coordinating services, and connecting clients to vital community resources.

You will collaborate with a diverse team of health and social service professionals, using creative problem-solving and a client-centered approach to ensure individuals have every opportunity to lead fulfilling, self-directed lives.

Day to Day :

• Build trusting relationships through empathetic, compassionate, and effective communication.
• Support Minnesota Health Care Program members in making informed health care decisions.
• Conduct assessments and coordinate services to support members’ health goals.
• Provide ongoing care coordination, including planning, referrals, monitoring, and advocacy.
• Collaborate with healthcare providers and community resources for comprehensive care.
• Advocate for members’ rights and assist with waiver coordination.
• Ensure compliance with agency guidelines and maintain accurate documentation across multiple systems.
• Deliver culturally sensitive care to individuals from diverse backgrounds.

Requirements :

Bachelor’s degree in Social Work, Psychology, Sociology, or a closely related field (e.g., Occupational Therapy, Physical Therapy, Speech-Language Pathology, Audiology, Recreational Therapy, Dietetics, Special Education, Rehabilitation Counseling, Nursing, or Human Services), plus at least one year of experience in service planning.
