Position Overview :
As a Care Coordinator, you will build trusting relationships with the individuals you serve while playing a key role in supporting their overall well-being and independence. Your responsibilities will include conducting assessments, developing personalized care plans, coordinating services, and connecting clients to vital community resources.
You will collaborate with a diverse team of health and social service professionals, using creative problem-solving and a client-centered approach to ensure individuals have every opportunity to lead fulfilling, self-directed lives.

Requirements :
Bachelor’s degree in Social Work, Psychology, Sociology, or a closely related field (e.g., Occupational Therapy, Physical Therapy, Speech-Language Pathology, Audiology, Recreational Therapy, Dietetics, Special Education, Rehabilitation Counseling, Nursing, or Human Services), plus at least one year of experience in service planning.
