Position Summary : \n\nWe are seeking a dedicated and experienced Director of Nursing (DON) to lead the clinical operations of our growing home health agency in Indiana. - Oversee day-to-day clinical operations and supervise all field and office clinical staff.\n
- Ensure timely and accurate completion and submission of OASIS assessments, including SOC, ROC, Recertification, and Discharge.\n
- Maintain compliance with federal, state (Indiana Department of Health), and accreditation standards.\n
- Participate in the development and implementation of policies and procedures.\n
- Conduct performance evaluations, training, and in-services for nursing and paraprofessional staff.\n
- Review clinical documentation for accuracy, completeness, and compliance with Medicare and Medicaid regulations.\n
- Cooperate with physicians, case managers, and other healthcare professionals to coordinate patient care.\n
- Participate in quality assurance / performance improvement (QAPI) initiatives.\n
- Serve as a clinical resource for staff and assist in complex clinical decision-making.\n
- Manage staff schedules to ensure appropriate coverage and efficient service delivery\n\nRequirements : \n
- Registered Nurse (RN) licensed to practice in the state of Indiana (unencumbered license).\n
- Minimum 2–3 years of DON or leadership experience in a home health setting.\n
- Extensive OASIS experience and understanding of CMS regulations.\n
- Strong knowledge of Medicare Conditions of Participation (CoPs) and Indiana state home health regulations.\n
- Experience with EMR systems\n
- Excellent leadership, communication, and organizational skills.\n
- Ability to work independently and manage multiple priorities.\n
- Must pass background check and drug screening per agency and state policy.\n\nPreferred Qualifications : \n
- BSN or MSN degree.\n
- Experience with home health accreditation surveys (e.g., CHAP, ACHC, or Joint Commission).\n
