HOME HEALTH - DIRECTOR OF NURSING, RN

Position Summary : \n\nWe are seeking a dedicated and experienced Director of Nursing (DON) to lead the clinical operations of our growing home health agency in Indiana. The ideal candidate will have strong leadership skills, a deep understanding of OASIS documentation, and experience in compliance with CMS regulations and Indiana state requirements.\n\nAbout Us : \n\nWe are a licensed and accredited home health agency committed to delivering compassionate, high-quality care to clients in the comfort of their homes. Our focus is on improving outcomes through clinical excellence and compliance.\n\nKey Responsibilities : \n

• Oversee day-to-day clinical operations and supervise all field and office clinical staff.\n
• Ensure timely and accurate completion and submission of OASIS assessments, including SOC, ROC, Recertification, and Discharge.\n
• Maintain compliance with federal, state (Indiana Department of Health), and accreditation standards.\n
• Participate in the development and implementation of policies and procedures.\n
• Conduct performance evaluations, training, and in-services for nursing and paraprofessional staff.\n
• Review clinical documentation for accuracy, completeness, and compliance with Medicare and Medicaid regulations.\n
• Cooperate with physicians, case managers, and other healthcare professionals to coordinate patient care.\n
• Participate in quality assurance / performance improvement (QAPI) initiatives.\n
• Serve as a clinical resource for staff and assist in complex clinical decision-making.\n
• Manage staff schedules to ensure appropriate coverage and efficient service delivery\n\nRequirements : \n
• Registered Nurse (RN) licensed to practice in the state of Indiana (unencumbered license).\n
• Minimum 2–3 years of DON or leadership experience in a home health setting.\n
• Extensive OASIS experience and understanding of CMS regulations.\n
• Strong knowledge of Medicare Conditions of Participation (CoPs) and Indiana state home health regulations.\n
• Experience with EMR systems\n
• Excellent leadership, communication, and organizational skills.\n
• Ability to work independently and manage multiple priorities.\n
• Must pass background check and drug screening per agency and state policy.\n\nPreferred Qualifications : \n
• BSN or MSN degree.\n
• Experience with home health accreditation surveys (e.g., CHAP, ACHC, or Joint Commission).\n
