Physical Therapist - Home Health

Full-time Physical Therapist.

We are looking for a Full-time Physical therapist to work in our Home Health and Hospice program. Our Clinic is part of a community-centered, non-profit organization that provides one-on-one PT treatments to every patient. Orthopedic knowledge and manual therapy experience are strongly preferred.

The position of the Physical Therapist is to provide professional physical therapy services according to the accepted PT practice in consultation with the referring physician for home health and hospice. To facilitate coordination of care between home care disciplines (SN, OT, ST, CNA, and MSW) as indicated.

Job Responsibilities must include, but not limited to :
