Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.

An experienced and compassionate Home Health Registered Nurse (RN) is needed to provide skilled nursing care in patients homes, delivering patient-centered and continuous care with clinical excellence. This full-time, 13-week contract position is based in Granite City, IL, and offers a professional environment where autonomy and organization are valued.

Responsibilities include :

• Conducting comprehensive assessments using OASIS and developing individualized care plans.
• Delivering nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitoring patient progress and communicating changes promptly with healthcare teams and physicians.
• Educating patients and families on disease management, treatment plans, medications, and preventive care.
• Maintaining accurate documentation using Home Care Home Base (HCHB).
• Collaborating with therapists, social workers, and other healthcare providers to coordinate care.
• Participating in case conferences, staff meetings, and continuing education.
• Complying with all regulatory and agency standards.
