Full-Time Tailored Care Manager

We focus on each individual's unique needs and deliver the highest quality services possible by ensuring that each individual receives the right support at the right time and frequency by caring qualified staff member in the most independent setting appropriate.
What We Need :
The primary responsibilities of the Care Manager are to promote whole person, integrated care by planning, coordinating, tracking, closing of gaps in care and monitoring care to individuals to achieve within the guidelines of the tailored plans.
Job Description :
- Actively engages with members receiving care management services through assessment, care coordination, health promotion, & comprehensive transitional care / follow up.
- Coordinates referrals by making appropriate referrals to community resources and empowering individuals to be responsible for their own healthcare and personal needs.
- Coordinates follow-up services for individuals with recent inpatient hospitalization or emergency department visit within 24 hours of discharge. Coordinates after care needs for transitions in care such as a change in housing or other life transitions.
- Coordinates crisis response and providing this as necessary, following InReach's policies related to crisis.
- Participates in agency on-call structure as required and 24 : 7 crisis response for members.
- Participates in post crisis team debriefing.
- Recognizes diversity; involves individual in his / her support plan; explains steps and communicates clearly.
- Interacts with each individuals with respect and dignity.
- Establishes relationships with integrated Care Team Members and Community Resources.
- Maintains Documentation; medical records, compliance quality, secures files, database backups, etc.
- Maintains professional and technical knowledge of this position and InReach's polices and procedures.
Qualifications and Licensing Requirements :
Bachelor's Degreeminimum 2 years of experience working with the I / DD populationMicrosoft Office and Excel experienceValid driver's license and safe driving recordHas access to reliable transportation 24 hours a day and be able to travel to the home of any customer at any timeHours : Monday-Friday 9 : 00am-5 : 00pm; hybrid opportunity with one in office day per week (Tuesday) flexibility needed to meet the needs of customers / family

Rate : Salaried $48,000 - $52,000
