Registered Nurse - Throughput

Job Description

The Throughput Nurse is responsible for facilitating efficient and safe discharge of inpatients. The nurse in this role facilitates patient flow and discharge for inpatient units. The position processes discharges, schedules follow up appointments, provides discharge instructions to patients and family members, and resolves discharge barriers. The position may also be expected to participate in patient flow and throughput activities by assisting with transition / hand-off from the Emergency Department to an inpatient unit. The position collaborates with Charge Nurse, Primary Nurse, Physicians, APPs, and Case Manager to coordinate discharge plans and remove barriers. The nurse in this role must be skilled in patient assessment and critical thinking and be able to work independently and will be responsible for ensuring safe and efficient patient discharge. The person in this role will be instrumental in driving quality and efficiency in throughput and decreased length of stay and must be proficient in recognizing changes in patient condition and be able to collaborate with care team to ensure safe discharge.

Responsibilities

• Assesses patient and family needs to provide quality care to assigned patients at time of discharge.
• Develops, discusses, and communicates a realistic plan of care for each patient in order to address all identified needs.
• Demonstrates the skills and judgment necessary to implement medical plan of care, nursing interventions, and procedures as necessary for the care of the patient.
• Evaluates the identified problems, care provided, and patient's responses are reassessed to meet patient and family needs and assure overall quality of care delivered and to ensure patient and family are equipped for appropriate discharge planning.
• Performs accurate documentation of nursing care and discharge teaching which are provided to ensure the integration of information for use by the healthcare team, to ensure quality care and safe discharge.
• Performs all responsibilities / duties required by the nursing division as defined in the scope of service, to assure that the unique nature of the client is addressed. This includes, but is not limited to the age of the client served.
• Maintains current knowledge / certification / licensure as appropriate.
• Ability to be flexible with shift scheduling to ensure the division utilizes the position most effectively.
• May be required to remain on campus immediately before, during, and after severe weather and / or disasters.
