About the Role :

As a Licensed Vocational Nurse (LVN) at Treviso Transitional Care , you will play a critical role in delivering compassionate and high-quality nursing care to our residents. Your primary focus will be to support the health and well-being of patients by administering medications, monitoring vital signs, and assisting with daily living activities under the supervision of registered nurses and physicians. You will collaborate closely with interdisciplinary teams to develop and implement individualized care plans that promote recovery and comfort. This role requires a strong commitment to patient advocacy, ensuring that each resident's needs and preferences are respected and met with dignity. Ultimately, your contributions will directly impact the quality of life and health outcomes of the individuals entrusted to our care.

Minimum Qualifications :

  • Current and valid Licensed Vocational Nurse (LVN) license in the state of Texas.
  • Completion of an accredited vocational nursing program.
  • Basic Life Support (BLS) certification.
  • Demonstrated ability to perform nursing duties in a long-term care or rehabilitation setting.
  • Strong communication and interpersonal skills.

Preferred Qualifications :

  • Experience working in a nursing home or rehabilitation facility.
  • Additional certifications such as IV therapy or wound care.
  • Familiarity with electronic health record (EHR) systems.
  • Training in geriatric nursing or chronic disease management.
  • Ability to speak multiple languages relevant to the resident population.
  • Responsibilities :

  • Provide direct nursing care to residents, including administering medications and treatments as prescribed.
  • Monitor and record vital signs, symptoms, and changes in patient conditions, reporting significant findings to the healthcare team.
  • Assist residents with activities of daily living such as bathing, dressing, and mobility support.
  • Collaborate with registered nurses, physicians, and other healthcare professionals to develop and update individualized care plans.
  • Maintain accurate and detailed patient records in compliance with facility policies and regulatory requirements.
  • Educate residents and their families on health maintenance and disease prevention strategies.
  • Ensure a safe, clean, and comfortable environment for residents by adhering to infection control and safety protocols.
  • Skills :

    The required skills enable the LVN to provide safe and effective nursing care by accurately administering medications, monitoring patient status, and documenting clinical information. Strong communication skills are essential for coordinating with healthcare team members and educating residents and their families. Preferred skills such as proficiency with electronic health records streamline documentation and improve care coordination. Additional certifications enhance the nurse's ability to manage specialized treatments like IV therapy and wound care, contributing to better patient outcomes. Multilingual abilities facilitate culturally sensitive care and improve resident comfort and understanding.
