As a Community Support Service Coordinator for HeartWell, you will coordinate community-based services for individuals with intellectual and developmental disabilities. As a coordinator, you will work with a team of other coordinators to ensure the participants are achieving their goals and working towards becoming active members in their community. It will be the responsibility of the coordinator to ensure employees are trained appropriately to work with the individuals we support. It would also be the coordinators responsibility to advocate for the individuals and to maintain accurate documentation needed for services per DDW standards.

- Participate as an Interdisciplinary team member for all individuals on your caseload. Work in conjunction with the individual and their team to identify current and future goals, development, and implementation of TSS and outcomes.
- Review written, and verbal reports / documentation related to daily progress and provide intervention or training when needed.
- Advocate and teach others to advocate for the individuals supported.
- Act as a positive role model to both individuals and employees.
- Provide community inclusion support and behavior management. Mentor and train CCS Coaches with daily functionality of this support process.
- Assist with hygiene, grooming, assist with medication delivery, and any other areas identified within the ISP.
- Review all medications assisted with during CCS provisions for current and correct usage and availability, this includes communication with the pharmacy and medication pick up as needed.
- Cover open shifts as needed and work in partnership with other Service Coordinators to meet the needs of all Individuals receiving supports.
- Provide transportation (using personal vehicle) and ensure provisions for transport are available via LSC’s on shift for the individuals served. Accessing the community is a priority and a necessity for those whom we support.
- Provide staffing coordination to meet state and agency required coverage to maintain health and safety.
- Identify social, volunteer, economical, recreational, and educational services in the community that will meet the individual’s needs in addition to pre-planned activities. Meet with CCS Coaches and the individual regularly to ensure implementation of this process.
- All activities related to employment goals will be identified as priority.
- Provide emotional support and feedback to the individual and their family if applicable.
- Evaluate employee’s ability to successfully meet the needs of the individual. Provide formal and informal feedback / evaluations and training to the employee.
- Actively Mentor and train employees within the role they currently hold as well as provide advancement opportunity through supporting the management training program.
- Complete all required trainings and work within all HeartWell Policies & Procedures.
- Maintain regular communication with the assigned nurse, Program Director and HR on all appropriate related matters.
- Active participation in all internal and external audits / surveys
Requirements :
Minimum- High School Diploma or equivalentCurrent and Valid Driver's LicenseSafre insured / registered vehicleCompleted a minimum of 1 year of human service management (preferred) OR experience is DD Waiver Management
