This position is posted by Jobgether on behalf of a partner company. We are currently looking for a Chart Deficiency Analyst in the United States .

In this role, you will play a key part in ensuring the accuracy, completeness, and compliance of medical records for inpatient, outpatient, and procedural areas. You will analyze chart deficiencies, reassign tasks to the appropriate providers, and verify that documentation meets regulatory, accreditation, and hospital standards. Working in a structured, fast-paced environment, you will combine analytical skills with attention to detail to support patient care and operational excellence. This position requires collaboration with healthcare providers, nursing teams, and health information management staff, contributing directly to accurate clinical records and improved healthcare outcomes. You will manage multiple workqueues, perform research, and implement solutions that maintain compliance and efficiency.

Accountabilities :

  • Review, verify, and correct chart deficiencies for inpatients, outpatient procedures, and hospital-based surgeries.
  • Analyze, reassign, and resolve declined deficiencies, ensuring the correct provider is accountable.
  • Validate completed deficiencies, confirming documentation is accurate and meets policy standards.
  • Manage workqueues, including analysis needed, done deficiencies, and final analysis, to maintain workflow efficiency.
  • Collaborate with nursing and HIM teams to ensure accurate provider assignment and proper handling of verbal or cosigned orders.
  • Maintain compliance with federal and state laws, accreditation requirements, and internal policies.
  • Identify missing documentation elements, assign deficiencies correctly, and maintain high productivity and quality standards.

Requirements

  • High school diploma required; additional certifications in health information management preferred.
  • Minimum 3 years of experience in HIM record processing or equivalent acute care setting.
  • Strong analytical, problem-solving, and clerical skills with attention to detail.
  • Proficient in reviewing, researching, and correcting medical record deficiencies.
  • Excellent verbal and written communication skills to effectively interact with providers and team members.
  • Ability to handle multiple tasks, manage workqueues, and meet productivity and quality targets consistently.
  • Familiarity with healthcare documentation standards, hospital policies, and compliance regulations.
  • Physical and manual dexterity skills, including hand-eye coordination, finger dexterity, and visual discrimination.
